Classroom Check-in Survey

Our classroom check-in survey is a quick, easy, and confidential way for educators to assess student well-being.

It’s vital that teachers, administrators, and other school adults foster a sense of belonging and community with one another and their students; make informed decisions about what children need to know and understand in order to feel safe; and focus on creating an environment where children can ask the questions that matter to them.

You can use the questions on this survey as they are, add or eliminate questions, or check in with students in another way, but try to gather information about the experiences your students currently face weekly or even daily. As you do, consider the importance of equity and each student’s access to services that might benefit them. Be sure to gather feedback from students about ways in which you and your colleagues can help students connect with you and one another.

Collecting data in this way can help you gauge students’ well-being and provide students with an opportunity to ask questions in a confidential way.

Note: You will be prompted to sign into your Google account to make a copy of the check-in survey when you click the link below.
